The test is performed with the patient’s knee starting in extension (straight) - in this position the tibia (shin bone) subluxes or sits forward. The knee reduces back to the normal position at 20-30 degrees of flexion with a positive pivot shift. It is a measure of instability after ACL injury and ACL reconstruction.
